29 CFR 1926.502(d)(17):The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. On or about April 5, 2024, the employer did not ensure that employees connected their harnesses to fall protection anchorage points correctly. Employees performing work at heights of 16 feet had their lanyards attached to the front side of their harnesses, exposing them to serious injury in the event of a fall. No abatement certification or documentation is required for this item.

29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training. On or about April 5, 2024, the employer did not certify that employees had been trained to recognize and minimize fall hazards. Employees were exposed to fall hazards in excess of 16 feet while performing roofing activities and lanyard not tied-off to d-ring on back. Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation. On or about April 5, 2024, employees engaged in residential roofing operations were exposed to eye injury hazards while operating pneumatic nail guns without eye protection. Employer did not ensure that employees that were engaged in framing activities utilizing pneumatic guns were utilizing safety glasses. No abatement certification or documentation is required for this item.
