29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): A) 148-152 West Westfield Ave Roselle Park NJ 07204 - An employee was elevated 20 feet above the ground while standing on wooden pallet(s) on the forks of a Genie GTH 1056 telehandler. The employee was fastening exterior wood sheeting to the new building under construction without guardrail or personal fall arrest system(s) in use; exposed to hazard of falling 20 feet. Operator(s) of powered industrial trucks, including the Genie GTH 1056 telehandler, were not trained and evaluated in their safe operation. Hazardous condition(s) were observed on or about April 4, 2024.
