29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Site: 378 Ave T, Brooklyn, NY 11223 a) On or about 4/5/2024, employees performed work from a 3-tier tubular welded frame scaffold that did not have guardrail systems installed on the outrigger platform. The end of the working platform on the scaffold was not guarded.
