29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l), with certification records that includes the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: (a): On or about April 5, 2024 at M&W Grinding, in the warehouse, the employer did not certify that a forklift operator had been trained and evaluated as required by paragraph (l) with certification records. An employee was operating Toyota Sit Down- Forklift Truck (Model: 12-EGCU25). Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
