29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about April 3, 2024, the employer did not ensure that energy control procedures were documented and utilized to control hazardous energy (electrical) to prevent unexpected start-up of the plasma cutting system when employees were engaged in servicing and maintenance activities, exposing employees to crushing hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. On or about April 3, 2024, the employer did not ensure that junction boxes located in the back area of workshop were provided with covers.
