Employee(s) worked without donning harness connected to safety lines as part of a personal fall arrest system; exposing them to the hazard of falling 20 feet.

Bridge St. Somerville, NJ 08876- Residential Roof;  On 04/05/2024, the employer failed to provide roofing crew employee(s) with training on recognizing roof edge fall hazards and their control through proper utilization of personal fall arrest system equipment; as well as on the requirements of the OSHA Standard itself, as it pertains to roofing activities.  Employee(s) worked atop the sloped roof of a two-story residential property replacing wood decking as part of roof replacement job.  The employee(s) worked at the roof edge without donning a harness and lanyard connected to safety line(s) anchored to the rooftop as part of a personal fall arrest system; exposing them to the hazard of falling 20 feet.

The side rails of aluminum extension ladder(s) leaned against the building, used for main roof access / egress, were not extended at least 3 feet above the roof eave; exposing employee(s) to a hazard of falling 20 feet while stepping on/off the top rung.
