The #1 Charge Crane has multiple deficiencies including but not limited to: a defective upper limit control switch and worn brakes. b) On or about 04/22/2024 at the site located at 3000 Milton Ave., Solvay, NY: Employees exposed to struck by hazards when utilizing the Hoist-2 Maintenance Shop (Forklift Repair) lifting device to lift a motor for repair weighing approximately 250 lbs. The Hoist-2 maintenance shop hoist had a broken safety latch. c) On or about 04/09/2024 at the site located at 3000 Milton Ave., Solvay, NY: Employees exposed to struck by hazards when utilizing the Hoist Sand Reclaim Belt lifting device to lift items during maintenance activities approximately 25 lbs.
