29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At a new multi-family construction site located at Lot 11, N Askew Ave, KC MO -- employees engaged in framing duties working below other employees working from heights were not wearing head protection. This exposed the employees to struck-by hazards in that tools, material and other debris could fall on them causing injury. This was documented on or about 4/3/2024.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a new construction multi-family dwelling located at Lot 11, N Askew Ave, KC MO -- employees engaged in framing duties were using pneumatic nail guns, skil-saw, and other small hand tools were not wearing eye protection. This exposed employees to getting debris in their eyes. This was documented on or about 4/3/2024.

29 CFR  1926.405(a)(2)(ii)(I):Flexible cords and cables used for temporary wiring were not protected from damage:  At a new construction multi-family site located at Lot 11, N Askew Ave, KC MO -- employees engaged in framing duties were using a flexible cord, also knows as an extension cord, that the outer insulation was torn and ripped exposing the internal wires. This created a hazard of electrical shock and/or burns from the wires exposed. This was documented on or about 4/3/2024.

29 CFR  1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed:  At a new multi-family construction site located at Lot 11, N Askew Ave, KC MO -- employee was engaged in framing duties using a step ladder, also known as an A-Frame ladder. The ladder was not in the open position as designed purpose was for. Rather, the ladder was in the closed position and leaning against the building. This exposed the employee to a fall hazard from the ladder not being used in the purpose for which it was designed. This was documented on or about 4/3/2024.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a new multi-family construction site located at Lot 11, N Askew Ave, KC MO -- employee engaged in framing duties was not using any kind of personal fall arrest system. This exposed employees to fall hazards greater than  12 feet to ground level.   Triple R Framing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1681836, citation number 1, item number 1 and was affirmed as a final order on 10/11/2023, with respect to a workplace located at 17345 W 170th St, Olathe KS.
