29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut: a. At the 3rd floor of Bld. #1 in 7280 Arlington Expressway, Jacksonville, Florida, on or about March 29, 2024, the employer exposed employees to an amputation hazard, in that the RIGID brand (Model R4514) portable table saw was being used without the blade protective guard in place.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a. At the 3rd floor of Bld. #1 in 7280 Arlington Expressway, Jacksonville, Florida, on or about March 29, 2024, the employer failed to report a work-related amputation injury of an employees within twenty-four (24) hours. The work-related amputation injury was reported to OSHA on April 4, 3024.
