29 CFR 1910.23(c)(9): The employer did not ensure that portable ladders that are used on slippery surfaces are secured and stabilized. On or about April 2, 2024, at the southside of a building, an extension ladder was set on wet asphalt and the ladder was not secured or stabilized when employees climbed down the ladder after performing HVAC work on the roof.
