29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about March 31, 2024, the employer did not ensure one or more methods of guards were designed and constructed as to prevent employees from having any part of their body in the danger zone during the operating cycle while operating the Portfolio Machine (Job No.: B1388 / SN: B-1388-03-00). Employees were exposed to amputation hazards while operating the machine located at 850 West Park in Union, MO.
