29 CFR 1926.1413(a)(1): A competent person did not begin a visual inspection prior to each shift the equipment is used and/or did not complete the visual inspection before or during that shift. The inspection must consist of observation of wire ropes (running and standing) that are likely to be in use during the shift for apparent deficiencies, including those listed in paragraph (a)(2) of this section. (a) On or about April 04, 2024, at 333 N New River Drive East, Fort Lauderdale FL 33301, the shift inspection did not identify apparent deficiencies to include, but not limited to significantly corroded, and cracked, pins and bolts, and improperly applied (tightened) end connections, exposing employees to a fall greater than 300 feet to the ground.

Before each crane component is erected, it must be inspected by a qualified person for damage or excessive wear. a) On or about April 04, 2024, at 333 N New River Drive East, Fort Lauderdale FL 33301, employer did not ensure pre inspection of each crane component including, but not limited to, the U-bolt (clamps/clips), bolts, pins, thimble, wire rope of the Terex Top Climbing System- Trolly Rail Guide (TRG) was adequately inspected by a qualified person for damage or excessive wear, exposing employees to a fall greater than 300 feet to the ground.
