29 CFR 1926.602(d):The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l). On or about April 5, 2024, and times prior thereto, employees prepared to erect preassembled rib structure beams and used a powered industrial truck driven by an operator not trained or certified by the employer. The beams were moved by an employee operating a powered industrial truck exposing workers to struck by hazards.
