On or about April 8, 2024, at the above addressed jobsite, an employee conducting a sewer connection to a sewer main, in a trench approximately seven (7) feet in depth did not have a safe means of egress.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about April 8, 2024, at the above addressed jobsite, an employee was conducting sewer main connection operations in a trench measured at approximately seven (7) feet in depth. The trench was not provided with cave in protection, thereby exposing the employee to cave in hazards.
