29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a) Residential home located on 892 Rocky Creek Hampton, GA 30228, Roof: On or about April 8, 2024, employees tasked with installing TPO roofing to a 4/12 pitched roof were not protected from struck by hazards, in that protective eyewear was not provided nor used while operating pneumatic nail guns at 150 PSI.

29 CFR 1926.501(b)(10):   Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a) Residential home located on 892 Rocky Creek Hampton, GA 30228, Roof: On or about April 8, 2024, the employer exposed employees to falls of up to 11-feet, 5-inches to the lower level, in that a fall protection system was not provided to employees engaged in roofing activities on a 4/12 pitched roof.
