Employees change out welding tips daily without locking out all machine energy sources, on or about April 8, 2024. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(f)(1)(ii):When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): (a): Facility -TRC welder - Employees were not removed from the machine when testing and cycling the machine during maintenance. An employee sustained a finger fracture and laceration when his finger was caught inside the moving parts during testing, on or about April 8, 2024. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
