29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   On or about April 9, 2024, employees were exposed to struck by hazards while using pneumatic nail guns to install shingles on a residential roof located at 3363 Buckhaven Drive, Richfield, OH 44286, without being required by the employer to wear any form of eye or face protection.  Selvin Meza was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1418571, citation number 1, item number 1 and was affirmed as a final order on 10/18/2019, with respect to a workplace located at 735 N Revere Road, AKRON, OH 44333.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about April 9, 2024, employees were exposed to a fall hazard of 18.6 feet above the concrete while installing shingles on the roof of a residential home located at 3363 Buckhaven Drive, Richfield, OH 44286, and were using insufficient forms of fall protection or not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure.  ARA Remodelers, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1679756, citation number 1, item number 1b and was affirmed as a final order on 08/24/2023, with respect to a workplace located at 4678 Talmadge Road Toledo, Ohio 43623.  Selvin Meza was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1418571, citation number 1, item number 2 and was affirmed as a final order on 10/18/2019, with respect to a workplace located at 735 N Revere Road, AKRON, OH 44333.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about April 9, 2024, employees were exposed to a fall hazard of 12 feet while completing roofing work on a residential home located at 3363 Buckhaven Drive, RICHFIELD, OH 44286. Employees were using an extension ladder to access the roof and the employer did not ensure it extended at least 3 feet above the upper landing surface.
