In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a. On or about April 9, 2024, located on the side of the Dumper 7, there was a metal junction box missing a cover, exposing employees to contact with the electrical conductors inside the box.  b. On or about April 9, 2024, located by the Hydroscan Tunnel, on the Exceptions area, there was a metal outlet box with a bent cover, exposing employees to the electrical wiring inside.
