29 CFR 1910.303(b)(2):Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. a. On or about April 9, 2029, the employer did not ensure that flexible cords used to power electrical equipment at the Hydroscan area were used in accordance with instructions included in the listing or labeling. The flexible cord used was plugged in series into a power strip plugged into the wall outlet providing power to the cords and the electrical equipment being used in the area. As a result, employees were exposed to electrical fire hazards.

29 CFR 1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a. On or about April 9, 2024, located on the side of the Dumper 7, there was a metal junction box missing a cover, exposing employees to contact with the electrical conductors inside the box. b. On or about April 9, 2024, located by the Hydroscan Tunnel, on the Exceptions area, there was a metal outlet box with a bent cover, exposing employees to the electrical wiring inside.
