29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  On April 9, 2024, employees of Twisted Brick LLC were performing masonry work on a frame scaffold located at a residential building located at 1617 Ashford Hill Court, Glencoe, Missouri.  The employer did not ensure the work platform of the third tier approximately 16 feet high was fully decked or planked. The employees were exposed to fall hazards to the lower level.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On April 9, 2024, employees of Twisted Brick LLC were working from a frame scaffold located at a residential building located at 1617 Ashford Hill Court, Glencoe, Missouri. The employer did not ensure the frame scaffold was free of potential fall hazards. Employees were not protected from falling by either guardrails or a personal fall protection system.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:  On April 9, 2024, employees of Twisted Brick LLC were performing tuckpointing operations at a residential building located at 1617 Ashford Hill Court, Glencoe, Missouri.  The employer did not ensure a fall protection system was utilized by employees on the roof of the building.  Employees working on the roof were exposed to fall hazards up to approximately 11 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access:   On April 9, 2024, employees of Twisted Brick LLC were performing tuckpointing operations at a residential building located at 1617 Ashford Hill Court, Glencoe, Missouri. The employer did not ensure a ladder or other means of safe access was used by employees at the jobsite.  Employees were climbing up the scaffold side frames and crossbraces to get to the working level.  Employees were exposed to fall hazards.

The employer did not ensure that the scaffolding system was inspected by a competent person for structural integrity and defects prior to allowing employees to perform masonry work from the scaffold system.

29 CFR  1926.452(c)(4): End frames of tubular welded frame scaffolding were not locked together vertically by use of pins or equivalent means in areas where uplift could occur:  On April 9, 2024, employees of Twisted Brick LLC were performing tuckpointing operations at a residential building located at 1617 Ashford Hill Court, Glencoe, Missouri.  The employer did not ensure the end frames of the frame scaffold were locked together to prevent uplift.  The employees working on the scaffold were standing on the end frames creating the possibility of uplift on the vertical uprights.  The employees were exposed to fall and struck by hazards if the scaffold was to collapse.
