29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Repair Shop area: On or about April 10, 2024, and at times prior to, the employer exposed employee(s) to fall hazards and did not ensure manufacturers prior approval before allowing employees to use a modification attachment on the forks in the form of a chemical tote cage with plastic tote removed to be used as a manlift baskets to lift employee up to 20 feet high to replace light bulbs with company owned PIT's. b) Yard area: On or about April 10, 2024, and at times prior to, the employer exposed employee(s) to struck by hazards and did not ensure manufacturers prior approval before allowing employees to use a modification attachment on the forks in the form of a metal slide with a welded ball hitch that did not have any information pertaining to manufacturer, or weight/load capacities to move boats around the yard with company owned PIT's.
