29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about 04/10/2024, in the warehouse of an establishment located at 2020 Seabird Way, Riviera Beach, Florida, employees were exposed to struck-by hazards due to the employer not ensuring each powered industrial truck operator had been trained, evaluated, and received certification prior to allowing the employee to operate the truck.
