LOCATION: The Jobsite at 829 West Roxbury Pkwy, Chestnut Hill, MA On or about April 9th, 2024, employees were exposed to fall hazards of up to 20-feet while working on site, where the employer did not have a safety program in place that provided for frequent and regular inspections of the site by a competent person.

LOCATION: The Jobsite at 829 West Roxbury Pkwy, Cestnut Hill, MA On or about April 9th, 2024 The employer exposed employees to fall hazard of 20 feet from the second floor to the pavement below, while working where fall protection had not been used or provided, as set forth in STD 03-00-001, the interim fall protection guidelines for residential construction.
