29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. At the jobsite located at 22403 W. 52nd Street, Shawnee, Kansas; employees using a portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 11 feet from the roof to the ground below. The side rails did not extend at least 3 feet above the landing surface of the roof. The side rails extended approximately 14 inches.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: At the jobsite located at 22403 W. 52nd Street, Shawnee, Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 11 feet from the eave of the roof to the ground below. Employees were not protected against fall hazards by utilizing guardrail systems, safety net system, or personal fall arrest system. Conejo Roofing and Guttering, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1716673, citation number 1, item number 1 and was affirmed as a final order on 03/19/2024, with respect to a workplace located at 9751 Shady Bend Cir, Lenexa Kansas.
