Gillespie Construction, LLC at jobsite located at 2565 Blanchard Ave. Findlay OH 45840, did not ensure that employees were protected from head injury hazards by protective hardhats while working directly below a raised powered industrial vehicle (rough terrain forklift), which was lifting prefabricated trusses onto the second level of a single family home new construction jobsite.

29 CFR 1926.405(a)(2)(ii)(I):Flexible cords and cables used for temporary wiring were not protected from damage: On or about April 10, 2024 the employer J. Gillespie Construction, LLC at jobsite located at 2565 Blanchard Ave. Findlay OH 45840, did not ensure flexible cords and cables were protected from damage and from vehicular traffic. Employees were exposed to electrical shock hazards of 120 volts, in that two flexible cords were not protected from damage, in that the onsite JCB 506B Loadall rough terrain forklift had driven over the two flexible cords numerous times on the jobsite.

Gillespie Construction, LLC at jobsite located at 2565 Blanchard Ave. Findlay OH 45840, did not ensure employees were protected from falls while performing construction truss installation and framing activities atop a low sloped residential wood framed structure. One employee preparing for and installing trusses on the commercial wood framed building were exposed to falls of 11 feet and 9 inches or greater fall on a stud wall walking- working surface to the ground below.
