29 CFR 1926.502(b)(1): The top edge height of top rails, or equivalent guardrail system members, were not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: a) Job site, 327 South Main, Pittsburgh, PA: On or about April 9, 2024, during light installation activities, the employer did not ensure that the parapet wall along the top of the building was tall enough to serve as a guardrail system in that the top of the wall was determined to be approximately 34 to 36 inches in height, where a fall hazard of up to approximately 15 feet or so existed.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that would enable each employee to recognize the hazards of falling and that trained each employee in the procedures to be followed in order to minimize these hazards: a) Job site, 327 South Main, Pittsburgh, PA: On or about April 9, 2024, prior to performing light installation activities, the employer did not ensure that an employee received appropriate fall protection training. An employee worked on a roof where the parapet wall was determined to lack adequate height to serve as a guardrail system and a fall hazard of up to at least approximately 15 feet or so existed.
