29 CFR 1926.102(a)(1):  The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Residential structure, various locations; On or about April 10, 2024, employee(s) were operating an Interchange Round Head Model NSR83-21 Pro pneumatic framing nailer and a Metabo HPT Model NR 90ADS1 pneumatic framing nailer to secure trusses without the use of appropriate eye protection.

29 CFR 1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  (a) South side of structure; On or about April 10, 2024, three Louisville Cross Step (Model FXS1508) 8-foot fiberglass ladders setup as straight ladders and used to access the 1st floor and/or top wall plate(s), did not extend at least 3 feet above the landing surface.
