29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Scaffold frame was used as a means of access:  a) 4701 Pine St, Philadelphia, PA 19143 - Exterior Scaffold, West side of building - On or about April 11, 2024, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the 2nd level of the scaffold platform, thereby exposing employees to fall hazards of approximately 13.5 feet.  b) 4701 Pine St, Philadelphia, PA 19143 - Exterior Scaffold, South side of building - On or about April 11, 2024, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the 1st level of the scaffold platform, thereby exposing employees to fall hazards of approximately 6 feet.

Employees worked on uninspected scaffolding performing masonry work, thereby exposing employees to fall hazards of approximately 6 feet to 13.5 feet.

Employee performed masonry facade work on the front (southern side) of the building from a scaffold platform that was not fully planked, thereby exposing employees to fall hazards of approximately 6 feet.   b) 4701 Pine St, Philadelphia, PA 19143 - On or about April 11, 2024, the employer did not ensure each platform on all working levels of scaffolds was fully planked or decked between the front uprights and the guardrail supports. Employee performed masonry facade work on the west side of the building from a scaffold platform that was not fully planked, which lacked three planks, thereby exposing employees to fall hazards of approximately 13.5 feet.

29 CFR  1926.451(e)(5)(i): Ramps and walkways 6 feet (1.8 m) or more above lower levels did not have guardrail systems that comply with subpart M of this part -- Fall Protection:  a) 4701 Pine St, Philadelphia, PA 19143 - On or about April 11, 2024, the employer did not ensure ramps and walkways 6 feet or more above lower levels had adequate guardrail systems. Employees used a walkway made of scaffolding planks which connected two scaffolds located around the southwest corner of a building to access work areas to perform masonry facade work approximately 13.5 feet from the ground. The walkway lacked adequate guardrails, thereby exposing employees to fall hazards of approximately 13.5 feet.
