29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a. On or about April 12, 2024, at the above addressed jobsite, employees performing commercial ironwork operations from a Sky Power JLG 600AJ aerial lift, approximately twenty-five (25) feet above the ground, were not wearing a personal fall protection system, thereby exposing the employees to fall hazards.

29 CFR 1926.1427(a)(1)(i): The employer did not ensure that, prior to operating any equipment covered under subpart CC the person operating the equipment was licensed by a government entity for operation of the equipment within that entity's jurisdiction:  a)??On April 12, 2024, at the above addressed jobsite, the employer operating a National Series 400 Truck mounted crane was not trained, certified/licensed, and evaluated in accordance with the requirements of 29 CFR 1926.1427, thereby exposing employees to struck by and crushed hazards.
