29 CFR 1910.213(c)(2): Each hand-fed circular ripsaw was not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator. A) In the woodworking shop there was a table saw that was missing a spreader behind the blade to make sure any wood being cut would not cause the blade to catch on the wood and throw the material back at the operator.

29 CFR 1910.213(c)(3): Each hand-fed circular ripsaw was not provided with non-kickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator. They shall be designed to provide adequate holding power for all the thicknesses of materials being cut. A) In the woodworking shop there was a table saw that was missing anti-kickback fingers to prevent the wood stock being cut from being thrown at the operator if the blade was caught the stock.

29 CFR 1910.215(b)(9): The distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceeded one-fourth inch. A) In the welding shop there was a bench grinder that was missing a tongue guard for a grinding wheel, with nothing to protect the operator from fragments of the wheel coming off and striking them.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. A) in the welding area, Oxygen and Acetylene cylinders were stored next to each other. The cylinders were not on a rack or being used on oxy/acetylene torch.
