29 CFR1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end: A) On or about 04/09/2024, at the north/west corner of 5th Street & 50th Ave. Long Island City, NY Employees working on a scaffold, referred to as a sidewalk shed, were exposed to a fall hazard. A support beam/Q-deck was displaced on the underside of the sidewalk shed by a truck. One end of the 8-foot plank was not supported. The employees were exposed to fall approximately 12-foot to the ground.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: A) On or about 04/09/2024, at the north/west corner of 5th Street & 50th Ave. Long Island City, NY Employees working on a scaffold, referred to as a sidewalk shed, were exposed to a 12-foot fall hazard. A support beam/Q-deck was displaced on the underside of the sidewalk shed by a truck. One end of the 8-foot plank was not supported. The bottom of the sidewalk shed was not inspected by a competent person for defects before employees worked on the top of the shed.
