29 CFR  1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained:   (a) On or about 04/12/2024 - 6401 Weaver Road, Anniston, AL, eye protection was not worn by employees operating pneumatic nail guns while laying asphalt shingles.  Ricardo Hernandez was previously cited for a  serious violation of this Occupational Safety and Health Standard  29 CFR 1926.95(a), which was contained in OSHA inspection number 1590573, citation number 1, item number 1 and was affirmed as a final order on 06/09/2022, with respect to a workplace located at 1479 Sanie Road, Odenville, AL.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  (a) On or about 04/12/2024 - 6401 Weaver Road, Anniston, AL, employees applying felt and shingles on a steep roof (4:12) were exposed to fall hazards of approximately 10 feet.  Ricardo Hernandez was previously cited for a  serious violation of an equivalent  Occupational Safety and Health Standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1590573, citation number 1, item number 2 and was affirmed as a final order on 06/09/2022, with respect to a workplace located at 1479 Sanie Road, Odenville, AL.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about 04/10/2024 - 6401 Weaver Road, Anniston, AL, the ladder used by employees to gain access to the roof did not extend 3 feet or more above the eave of the roof.  Ricardo Hernandez was previously cited for a  serious violation of this Occupational Safety and Health Standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1590573, citation number 1, item number 3 and was affirmed as a final order on 06/09/2022, with respect to a workplace located at 1479 Sanie Road, Odenville, AL.
