29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:   a) 147 Quigley Blvd, New Castle, DE 19720: On or about April 15, 2024, the employer failed to provide refresher training to 3 (Building Equipment Mechanics) employee(s) tasked with operating/maintaining USPS owned power industrial trucks (PITs). The Building Equipment Mechanics were tasked to operate/service equipment such as but not limited to fork trucks, pallet jacks, motorized walkie/rider hand trucks, and other specialized industrial trucks powered by electrical motors or internal combustible engines.  b) 147 Quigley Blvd, New Castle, DE 19720: On or about April 15, 2024, the employer failed to provide refresher training to 3 (2022) employee tasked with operating USPS owned or rented power industrial trucks (PITs). The operators were tasked to operate equipment such as but not limited to fork trucks, pallet jacks, motorized walkie/rider hand trucks, and other specialized industrial trucks powered by electrical motors or internal combustible engines.  c) 147 Quigley Blvd, New Castle, DE 19720: On or about April 15, 2024, the employer failed to provide refresher training to 1 (2021) employee tasked with operating USPS owned or rented power industrial trucks (PITs). The operators were tasked to operate equipment such as but not limited to fork trucks, pallet jacks, motorized walkie/rider hand trucks, and other specialized industrial trucks powered by electrical motors or internal combustible engines.  d) 147 Quigley Blvd, New Castle, DE 19720: On or about April 15, 2024, the employer failed to provide refresher training to 3(2020) employees tasked with operating USPS owned or rented power industrial trucks (PITs). The operators were tasked to operate equipment such as but not limited to fork trucks, pallet jacks, motorized walkie/rider hand trucks, and other specialized industrial trucks powered by electrical motors or internal combustible engines.  e) 147 Quigley Blvd, New Castle, DE 19720: On or about April 15, 2024, the employer failed to provide refresher training to 3 (2019) employees tasked with operating USPS owned or rented power industrial trucks (PITs). The operators were tasked to operate equipment such as but not limited to fork trucks, pallet jacks, motorized walkie/rider hand trucks, and other specialized industrial trucks powered by electrical motors or internal combustible engines.  f) 147 Quigley Blvd, New Castle, DE 19720: On or about April 15, 2024, the employer failed to provide refresher training to 14 (2017) employees tasked with operating USPS owned or rented power industrial trucks (PITs). 29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  a) 147 Quigley Blvd, New Castle, DE 19720 (Battery charging room): On or about April 15, 2024, employees were not able to access or open an exit route door from the inside as a twist-turn operated dead bolt lock was attached to the door. These locks expose employees to fire hazards. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.36(d)(1), which was contained in OSHA inspection number1328027 citation number 1, item number 1 and was affirmed as a final order on 5-7-2019, with respect to a workplace located at 560 Huyler Street South Hackensack, New Jersey, 07606.
