29 CFR 1926.453(b)(2)(v): A safety harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a)Mydatt Services, Inc. dba Block by Block, Inc. at 332 South Phillips Avenue, Sioux Falls, SD 57104: On or about and at times prior to April 15, 2024, the employer did not ensure that an employee was protected from falls by a personal fall arrest system while conducting commercial decorative light removal activities while working from a MEC Aerial Platform BOOM 45-J, serial number 14700360. This condition exposed the employee to fall and ejection hazards while working from the aerial lift.

This condition exposed an employee to fall and ejection hazards while working at heights greater than 10 feet.
