29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Parts Department:  On or about April 04, 2024, the employer exposed employees to amputation hazards, in that one or more methods of machine guarding were not provided to protect operators from the point of operation of the "Wysong and Miles Co." Model C-96 roller machine (Serial Number PR19-107) where employees were roll forming sheet metal.

29 CFR  1910.219(d)(1):  Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):  a.  Parts Department:  On or about April 15, 2024 the employer exposed employees to amputation hazards, in that effective machine guarding was not provided to protect operators from the power transmission pulleys located between 1 foot above the floor and 2 feet, 8.5 inches above the floor on the "Wysong and Miles Co." Model C-96 roller machine (Serial Number PR19-107) where employees were roll forming sheet metal.

Parts Department:  On April 4, 2024, the employer exposed an employee to bloodborne pathogens hazards, in that the employer did not offer an employee the Hepatitis B vaccination within 10 days of performing tasks not previously assigned and which resulted in occupational exposure to blood or other potentially infectious materials where an employee cleaned up blood following an amputation incident at the "Wysong and Miles Co." Model C-96 roller machine (Serial Number PR19-107).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)  Welding Area:  On or about April 15, 2024, and at times prior, the employer exposed employees to respiratory irritation hazards, in that hazard communication program training was not provided to welders who perform work with materials including "Industrial Welding Supply" compressed propylene, compressed oxygen, "Hobart Brothers LLC USA" FabCOR 86R welding wire, "Victory Welding Alloys" ER 70S-6 welding wire, "Oxford Alloys, Inc." 70S-6 welding wire, and "Bohler QS-6 welding wire where employees perform welding for metal shop fabrication.
