29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about April 16, 2024, Infinity Solutions Ohio LLC located at 152/154 E. Township Rd. 1178 Tiffin OH 44883, did not ensure subcontracted employees were protected from head injury hazards by protective hardhats while working directly below overhead roofing work, including shingle tear off using hand tools and shovels. Two employees were exposed to struck-by hazards in that they were working on the ground level cleaning up, and raking up torn off shingles, directly under ongoing overhead roofing work.

Township Rd. 1178 Tiffin OH 44883, did not ensure subcontracted employees were protected from falls while performing residential roofing activities atop a 4:12 pitch sloped residential roof. Nine employees were exposed to fall hazards of 10 feet or greater from the eve to the ground.
