Liquified petroleum gas (LPG) cylinders, that were not in use, were not secured to protect against abrasion or strikes by heavy machinery. 29 CFR  1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  At the  shop area, employee(s) engaged in grinding activities are exposed to struck-by and caught in hazards in that the tongue guard was adjusted over 1/4 of an inch from the grinding wheel. 1) At the Northeast side of the welding shop a bench grinder was 5/8 of an inch from the grinding wheel. 2) At the North side of the preparation line/room a bench grinder was 1 inch  from the grinding wheel.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  Employees in the welding shop were exposed to chemicals when using unlabeled chemical containers. Unlabeled spray bottles containing Weld Anti-Splatter was found throughout the welding shop. According to the safety data sheets, it causes mild skin irritation and serious eye damage.
