29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Industrial Screen & Maintenance Inc.: On or about, April 16, 2024, and at times prior, employees were exposed to caught-in hazards and struck by hazards while operating CNC Computer Lathe Machines, in that the safety-interlocks for the loading doors were inoperable.  Employees were exposed to hazards created by the rotating and moving parts.   (b) Industrial Screen & Maintenance Inc.: On or about, April 16, 2024, and at times prior, employees were exposed to caught-in hazards and stuck-by hazard while performing daily operations utilizing CNC Lathe Machines throughout the shop floor that were not equipped with guards to protect employees from the hazards created by the spinning chucks located on the outside of the machine.   (d) Industrial Screen & Maintenance Inc.: On or about, April 16, 2024, and at times prior, employees were exposed to laceration and amputation hazards while operating a W.F. Wells Bandsaw that was not equipped with a guard to minimize employee exposure to the saw.
