29 CFR 1910.30(a)(3)(iii): The employer did not train each employee in the correct procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems that the employee uses: a) Warehouse, Isle 241 A-5 - On or about April 17, 2024, an employee was operating a Raymond Model 550- OPC30TT order picker to retrieve product for an order and the employee fell approximately 12 feet to the ground below. The employee was not provided with fall protection training. The Hills & Marks LLC DBA Hills & Marks, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.30(a)(3)(iii), which was contained in OSHA inspection number 1557914, citation number1, item number 1 and was affirmed as a final order on July 14, 2022, with respect to a workplace located at 1997 New York 5S, Amsterdam, NY 12010.

29 CFR 1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace: a) Warehouse, Isle 241 A-5 - an employee was operating a Raymond Model 550- OPC30TT order picker to retrieve a product for an order and the employee fell approximately 12 feet to the ground below. The employee was not provided with training for operation of the order picker. The Hills & Marks LLC DBA Hill & Markes, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 910.178(l)(2)(ii), which was contained in OSHA inspection number 1557914, citation number 1, item number 2a and was affirmed as a final order on July 14, 2022, with respect to a workplace located at 1997 New York 5S, Amsterdam, NY 12010.
