29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) Building C, Unit 17: On or about April 4, 2024, employees using pneumatic nail guns to secure shingles to a roof, were exposed to the hazards of flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   a) Building C, Unit 17: On or about April 4, 2024, and at times prior to; employees working on a 7:12 pitched roof installing shingles were exposed to a fall hazard approximately 25-30 feet to the concrete sidewalk below.

29 CFR  1926.502(d)(17):The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  (a) Building C, Unit 17: On or about April 4, 2024, and at times prior thereto, employees performing work on a 7:12 pitched roof and were incorrectly attached to the fall restraint system being used.  The rope grab lanyard, when in use, was attached on the front of the harness on the leg.

29 CFR  1926.503(c): The employer did not retrain affected employees  who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section: Note:  Circumstances where retraining is required include, but are not limited to, situations where (1) Changes in the workplace render previous training obsolete; or (2) Changes in the types of fall protection systems or equipment to be used render previous training obsolete; or (3) Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill.  a) Building C, Unit 17: On or about April 4, 2024, employees were exposed to falls, ranging in height from approximately 25-30 feet, while involved in roofing activities. The rope grab lanyard, when in use, was attached on the front of the harness on the leg.
