29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift: a) Front side of the house - On or about April 17, 2024, an employee was exposed to a 10-foot fall hazard while cutting water oak tree branches from an elevated bucket without the use of personal fall protection.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. (a) Front side of the house - On or about April 17, 2024, employees were working on the ground with no hard hats while roof work was being done above at 8 feet and 2 inches, exposing them to struck by hazards.
