29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about April 17, 2024, at a commercial construction site located at the intersection of Cypress Rosehill Road and Holderrieth Road in Tomball, Texas, employer failed to ensure a ladder or other safe means of egress, were located in a trench excavation deeper than 4 feet, while two employees were working in the trench excavation. The Clearwater Utilities, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 1559393, citation number 2, item number 1 and was affirmed as a final order on December 7, 2021, with respect to a workplace located on Holderrieth Road, Harris County, Texas.

On or about April 17, 2024,at a commercial construction site located at the intersection of Cypress Rosehill Road and Holderrieth Road in Tomball, Texas, employer failed to ensure that employees were protected from excavated or other materials and equipment at least 2 feet from the edge of the excavations, that could pose a hazard of falling or rolling into the excavation.
