29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a)  On or about April 17, 2024, employees were exposed to a fall hazard of approximately 20-feet and greater while installing roof underlayment on a roof without using fall protection.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper level landing surface the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  a) On or about April 17, 2024, employees were exposed to a fall hazard of approximately 11-feet while using an aluminum extension ladder that was not set three feet above the roof landing.
