29 CFR  1926.252(a):An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  SW corner of 1st and 4th Floor: On or about April 17, 2024 employees were exposed to struck by hazards from material being thrown out the fourth floor balcony to the exterior of the building.

29 CFR  1926.501(b)(14):Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening.  SW corner of 1st and 4th Floor: On or about April 17, 2024 employees were exposed to fall hazards of 10 and 44 feet while throwing waste material from the unguarded balconies.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours.  Jobsite: On or about April 17, 2024, Occupation Safety Compliance Officer requested copies of the OSHA Forms 300 (Log of Work-Related Injuries and Illnesses) and OSHA's Form 300A (Summary of Work-Related Injuries and Illnesses). The requested copies were never provided by the employer. Bella Contracting Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.40(a), which was contained in OSHA inspection number 1532979, citation number 1, item number 1 and was affirmed as a final order on December 17, 2021 with respect to a workplace located at 189 Dayton Avenue Passaic, NJ  07055.
