29 CFR  1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:  a) Worksite - On or about April 17, 2024, employees were performing working on a Alum-A-Pole pump jack scaffold with a Werner platform approximately 29 feet above the ground. The platform was approximately 28 feet in length, the cantilevered end located below the access window was extended approximately 36 inches over the support. Employees were exposed to fall hazards.

29 CFR  1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed such that the components fit together with force and the scaffold's structural integrity was not maintained by the user:  a) Worksite - On or about April 17, 2024, employees were exposed to fall hazards of approximately 29 feet due to the Alum-A-Pole pump jack scaffold was erected with intermixed parts that included a Werner extension ladder for a top rail and a Werner platform that were both not properly secured with the chain system in place for Alum-A-Pole.

29 CFR  1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):   a) Worksite - On or about April 16, 2024, employees erected an Alum-A-Pole pump jack scaffold within approximately 6.5 feet of an uninsulated power line with voltage of 13.2kV phase to phase and 7.62kV phase to ground exposing employees to electrocution hazards. The erection of this scaffold was within the minimum approach distance of 10 feet.  b) Worksite - On or about April 17, 2024, employees worked from and used an Alum-A-Pole pump jack scaffold within approximately 5 feet from an uninsulated power line with voltage of 13.2kV phase to phase and 7.62kV phase to ground exposing employees to electrocution hazards. The use of this scaffold would be within the minimum approach distance of 10 feet.
