29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a) 625 Hamburg Turnpike/Roof: A means of access was not provided for the employees utilizing a Sunbelt scissor lift that was approximately 3 ft below a pump jack scaffold and approximately 5ft below a roof. The employees were performing roofing work, on or about 4/16/2024.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. c) 625 Hamburg Turnpike/Roof: Employee were not utilizing a personal fall arrest system or other means of fall protection when exposed to a fall hazard of approximately 20ft, on or about 4/16/2024.
