29 CFR  1926.451(b)(2)(i): Each ladder jack scaffold, top plate bracket scaffold, roof bracket scaffold, and pump jack scaffold was not at least 12 inches (30 cm) wide:  a) On or about April 19, 2024, employees were exposed to fall hazards from working on a scaffold that was missing multiple planks throughout the walking working surface and being less than 12 inches wide. This exposed employees to a fall distance of 12 feet 4 inches to the ground below.

29 CFR  1926.451(f)(3):Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a) On or about April 19, 2024, employees were exposed to fall hazards while working from a scaffold that had not been inspected by a competent person before the work shift began. The scaffold was missing multiple planks, had multiple protruding nails,  and protruding boards.

Employees conducting work on the scaffold were exposed to fall hazards  greater than 10 feet in height without fall protection, the scaffold was missing multiple planks throughout the walking working surface, and had multiple protruding nails and boards.
