29 CFR  1926.451(c)(2):Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates and mud sills or other adequate firm foundation:  Location: 42 Lenox Ct., Montville, NJ 07045  a) The employer did not ensure both legs of a scaffolding structure bore on an adequately firm foundation. The right leg of a two-legged pump jack scaffolding structure was resting in the dirt/mud without a mudsill or an acceptable firm foundation.  Violation occurred on about April 19, 2024.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   Location: 42 Lenox Ct., Montville, NJ 07045  a) The employer did not ensure that employees were protected from falls.  An employee engaged in residential siding activities worked on a pump jack scaffolding structure at a height of 28 feet from the immediate landing surface was exposed to fall hazards for not having any means of fall protection systems, guardrails or safety net systems.  Violation occurred on or about April 19, 2024.

Location: 42 Lenox Ct., Montville, NJ 07045  a) The employer did not ensure that the poles of a pump jack scaffolding structure were also securely attached to the structure at the bottom, exposing an employee to hazards associated with a 28-foot fall.
