On or about April 19, 2024, at the above addressed jobsite, employees using pneumatic nail guns were not wearing safety glasses, thereby exposing the employees to hazards associated with eye injuries.

On or about April 19, 2024, at the above addressed jobsite, employees performing residential roofing operations, thirteen (13) feet ten (10) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder's side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed and/or was not secured at the ladder's top:  a.  On or about April 19, 2024, at the above addressed jobsite, the portable ladders used to gain access to the roof were not extended at least three (3) feet above the upper landing surface and were not secured at the tops of the ladders, thereby exposing the employees to fall hazards.
