29 CFR 1926.652(b)(2):Maximum allowable slopes, and allowable configurations for sloping and benching systems, was not determined in accordance with the conditions and requirements set forth in Appendices A and B to this subpart. (a) Job site, 815 White Cloud Road, Apollo, PA 15613, On or about April 19th, 2024, employees were not protected from cave in's while working in a 7' 2" deep excavation in order to install three heli-piles inside of the excavation.
