29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. Location: 72 S Essex Ave, Orange NJ 07050. a) The employer failed to report within 24 hours an in-patient hospitalization. An employee engaged in the dismantlement of a scaffold was admitted to hospital and remained overnight after he slipped, fell and suffered a broken arm and head injuries. Violation occurred on April 10, 2024
